PCMH Recognition for Both Locations!

CCAPeds has been formally Recognized by The National Committee for Quality Assurance (NCQA) as being a Patient-Centered Medical Home (PCMH) for our Sarasota and North Port Offices!

"The Patient-Centered Medical Home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams.

Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care."

CCAPeds has worked to be fully Recognized as PCMH and earned Recognition status (effective 4/3/2015) and maintained this status by re-certification at annual checkpoints and ongoing third party monitoring. We have achieved Recognition status for our North Port Office (effective 9/4/2020) within ten months of opening our doors.

We plan to continue pursuing Recognition annually for both locations in our ongoing effort to provide excellent care and healthcare coordination.
