• We are only accepting patients who plan to follow the Vaccine Schedule published by American Academy of Pediatrics as stated HERE.

Call our office at 941-955-5191 to set up your child's account. We will discuss the following: 

1. Please be prepared to share all the child's information to our Front Office to register a new account.

2. We will need the Insurance and Guarantor Information to verify eligibility and any amounts owed at time of service.

3. We will ask if you have reviewed the Financial Page and are willing to follow the AAP Vaccine Schedule.

4. You will be advised to fill out the above Medical Release Form and to collect your child's Vaccine Records for their chart.
